If you are coming from a distance and plan to have a reversal in one trip, please send me the reports or results of the following:
 

	Your operative note
	A pathology report when available
	Preoperative evaluation as covered at testing.
	A list or note of your medical conditions, medication or treatment.
	A letter from your physician clearing you for surgery if you have any medical conditions.

Call 901 347-8331 to set up an appointment and the surgery.

You will need to send a deposit before being scheduled if the surgery is to be done on the same trip. Betsy can coordinate that.  She is at (901) 347-8331.  Tuesday, Wednesday and Friday are usually the best days to call.  Dr. Martin is out of the office in surgery and Betsy has more time available.  Dr. Martin is usually in the office Monday and Thursday.
